Improving Patient Outcomes
A computer programme is improving the delivery of care to patients and their families by increasing their involvement in the treatment they receive.
The Clinically Integrated System (CIS Model) brings together practices which have strong evidence to support their use, with various treatments and patient outcomes.
The model tracks each patient's progress to confirm the treatment is effective and to enable health care professionals to identify any problems and work together to solve them.
For the patient it means their treatment history and details are recorded on a computer, they are given a copy of the information and can access information about their treatment. It also means they are asked about how they think the treatment is working for them.
The development of the CIS Model has meant a fundamental change in the way that health professionals have previously worked. As the Model is computerised, new systems and processes have been developed to help ensure a more seamless approach to patient care delivery.
The project's core is to find ways to improve care of patients and their families/ whanau and encouraging their active participation in the treatment process.
